In Chicago, most kids with asthma or food allergies don't have a health management form on file at school, a new study shows. On the rare occasions when a child has died at school from an acute allergy attack or asthma attack, the children usually did not have a health management plan, or a so-called 504 Plan, on file, said Dr. Ruchi Gupta of the Center for Community Health at the Feinberg School of Medicine at Northwestern University in Chicago.
